Project Scope: Featuring: MFT 350 IP3 Feeder w/ SLD Open Top Vacuum Discharge and Impresso Labeler to feed and label blister and sleeve cards automatically. The feeder separates your products from an inclined stack. A Pneumatic Shaker is mounted onto the rear support assembly of the feeder to reduce friction between the products as they are fed and to assist with product flow. Once separated, the products are transferred onto the open top vacuum discharge of the feeder. As the products traveling on the discharge, a product sensor detects the product and signals the Impresso labeler to dispense a pre-printed label onto the product. The labeled product exits the discharge onto a customer supplied conveyor for further processing. System Notes: -RFQ as submitted has requested imperial measurements. MFT's standard is metric. -The stack height of the feeders' infeed magazine cannot be guaranteed (and was not tested) and will vary from product to product. -We will need to receive a larger stack of (EZ Start product, 82x313mm)to determine if it can be fed automatically (not currently included in this proposal). -The HS 103 - High Speed 3 Axis Labeling machine, equipped with Continuous Motion Accutipping, features an encoder that monitors the speed of the base conveyor matching the dispense speed of the label. -The HS 103 labeler has a separate motor for the unwind, drive, and rewind that is compatible with your short length labels. Set Up and Approval Note: -MFT will set-up and test three (3) products with one (1) label combination each. Additional set-ups are available as an option. -MFT has only tested one (1) product/label combination so the throughput for any additional product/label combinations will be determine when we actually test. MFT cannot guarantee the rate(s) of the other products having not tested them prior to this quote. -MFT will provide a one (1) eight-hour day Machine Acceptance with this proposal Notes: -This proposal is "formal" having received and tested sample products and labels. A video has been provided to the customer. -No URS, Specifications or FAT documents have been submitted with this proposal -Any URS, Specifications or FAT documents submitted during the quote or order phase will default to a requote -This proposal is the sole and governing document for this project -This proposal assumes that the customer will integrate, install and test the system at their facility -MFT is not responsible for pinch points created by the Integration of MFT supplied equipment and the Clients equipment (i.e.: feeders, labelers, conveyors, lugs, etc.). It is the sole responsibility of the client to address and add any additional guarding to meet local and federal OSHA requirements. -Standard MFT components are being integrated to make a "system". This proposal does not include "system" guarding. Guarding should be discussed with your MFT Representative to assure the system as proposed meets your project's requirements. MFT can option guarding per your projects’ requirements. -Any information provided after the order is considered a change in "Scope". A "scope” change quote will be provided to accommodate any/all additions and all changes will be billable to the customer. -This proposal does not include a system drawing but is available as an option -This proposal does not include MFT customer on-site training and/or installation assistance Labeler Notes: -All labels are fed wide edge leading per customer. -Label placement accuracy will be verified once both product and label samples have been submitted for testing and/or review. -MFT assumes that the customer has pre-qualified both the label material and the product for compatibility -Typical placement accuracy is ±2mm to ±3mm and is exclusive of product and/or label variations -MFT is not responsible for the RFID chip and any breakage (chip detaching from antenna) that might occur while being handled and/or dispense on the MFT labeler. MFT assumes that the RFID chip has been tested for automated dispensing. -Label supply roll = 20" OD x 3" ID Core -The paper label liner must be free of cuts, nicks and deep die-line cuts to be successfully applied at the speeds as quoted -Product flow is left to right per the operator facing the controls of the feeder. -Discharge height is 36" ±2"

Want samples sent back. 10/18: got labeler running at speed, need to adjust for placement on product now - CJ 10/20: wiring in the product sensor Cody and Kevin will start on; use existing create ??? check on it 10/16: Moved from Production to Integration - JL Added product samples information and photos; all photos in project docs; throughout speeds and shipping method need to be determined. -KSJ 10/11: checked build to dimensions and all match; I signed a layout indicating "good" and initialed -KSJ 10/10: In production. No shortages. 10/9: Change labeler from P/N9432005 LH to P/N9432004 RH; rev'd the Work Order and updated the financials; Josh to pull parts to make the change -KSJ 10/3: Feeder made it through 2nd burn in. Waiting on shortages. (purple frame). Prior to ship payment will arrive tomorrow. 9/26: Jobs are pulled and brought to Production. -RC 9/24: Waiting on parts to come in 9/19: Received a pallet of samples for test and run off. Waiting on some parts. 9/18: Added product photos; added product sample description information; saved photo ion Project/Docs; added label placement specifications; gave example samples to Sales (Gary); -KSJ 9/17: Received by Micah two (2) products and one (1) label size for set-up and testing 9/12: No change 9/5: No change 8/29: Waiting for parts to arrive. Latest date 10/9. Samples should arrive week of Sept. 23rd 8/27: All samples in-house are on Work Order Shelf -KSJ 8/22: Processed, waiting to order parts. 8/15: Processing currently 8/8: On procurement desk to process 8/6: Updated and reviewed Work Order; sent to Procurement for review/release -KSJ 8/5: Added customer Approved/Signed Drawing to Doc file. - GS 8/1: In ME.
